Vision insurance is available to eligible employees and their dependents. Coverage will begin the 1st of the month after 30 days of employment. Monthly rates are listed on the Enrollment/Election Form. Vision benefit information is available online at http://www.vsp.com/. Open enrollment is held in the spring of each year with an effective date of May 1st. Dependent children are covered to age 26.
1. Choose a VSP doctor at http://www.vsp.com/ or call 1-800-877-7195.
2. Make an appointment and tell the doctor you are a VSP member.
3. That's it! No ID cards or filling out claim forms.
