General Information
Coverage is provided to all eligible employees. Employees are covered after 30 days of employment at no cost to the employee.
An enrollment form is not necessary.
The elimination period is 90 calendar days of total or partial disability. The monthly benefit is the lesser of: (a) 66 2/3% of your basic monthly earnings, less other income benefits: or (b) $5000, the maximum monthly benefit, less other income benefits.
