Basic Supplemental Life
General Information
Basic Life Insurance Benefits: Active Public School employees who participate in any of the health plans sponsored by the State and Public School Employee Health Insurance will be automatically enrolled in $5000 of Basic Group Term Life and AD&D coverage with USAble Life. A Beneficiary Form should be completed and returned to Conway Public Schools Insurance/Benefits office.
Supplemental and Dependent Life Insurance is also available to employees who participate in any of the health plans sponsored by the State and Public School Employee Health Insurance. An Enrollment Application and Beneficiary Form should be completed and returned to Conway Public Schools Insurance/Benefits office.
